Official “Senior Circle Member Survey Contest” Rules
1) NO PURCHASE NECESSARY
No purchase of any product is required for participation. Senior Circle members can enter online at www.seniorcircle.com by completing a member survey. Non-members can submit a postcard entry (details below). ONLY ONE ENTRY PER PERSON. Multiple entries per person will not be accepted and may disqualify the entrant.
2) CONTEST DURATION
Online survey entries will be accepted from 12:01 a.m., October 14, 2008 to 11:59 p.m., November 21, 2008. All non-member entries must be postmarked by November 21, 2008.
3) ELIGIBILITY
This contest is open to all legal residents of the United States throughout the term of the Contest, who, at of time of entry, are 18 years of age or older.
The following are NOT eligible to enter or win this promotional contest: Any person employed by, or related to an employee of, Senior Circle, Community Health Systems, their affiliates, subsidiaries, affiliated hospitals or other entities under contractual agreement with Community Health Systems or Senior Circle.
Mail-in entries may not be mechanically reproduced. Electronic entries must be received by the SeniorCircle.com server no later than Contest End Date, and mail-in entries must be postmarked by Contest End Date and received by seven days after Contest End Date.
Sponsors are not responsible for and shall not be liable for late, lost, misdirected, or unsuccessful entries, either online or through the US Postal System.
All entries become the exclusive property of contest Sponsors, and will not be acknowledged or returned.
4) LIMITATIONS
You may enter Contest in one of two ways:
1. Senior Circle members may log on to www.seniorcircle.com and follow the instructions to complete a member survey to enter the contest. These instructions require the entrant to complete a survey in its entirety, and click the "SUBMIT" button to submit a valid entry. NO AUTOMATED ENTRIES WILL BE ACCEPTED.
Or
2. Legibly hand-print on a 3" x 5" card the words "Senior Circle Member Survey Contest," along with your name, street address, city, state, zip code, e-mail address (if you do not have an e-mail address, print "no e-mail address") and telephone number. Mail the entry, through the US Postal System, to:

5) ENTERING THIS CONTEST:
By entering, participants agree to be bound by these Official Rules and the decisions of Senior Circle which shall be final. Senior Circle, Community Health Systems, their parent companies, affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, and legal advisors, are not responsible for and shall not be liable for:
(1) late, lost, delayed, damaged, misdirected, incomplete, illegible, unintelligible, or postage-due entries;
(2) telephone, electronic, hardware or software program, network, Internet, or computer malfunctions, failures, or difficulties of any kind;
(3) failed, incomplete, garbled, or delayed computer transmissions;
(4) any condition caused by events beyond the control of the Sponsors that may cause the Contest to be disrupted or corrupted;
(5) any injuries, losses, or damages of any kind arising in connection with or as a result of the prize, or acceptance, possession, or use of the prize, or from participation in the Contest; or
(6) any printing or typographical errors in any materials associated with the Contest. Senior Circle and/of Community Health Systems reserves the right, in its sole discretion, to suspend or cancel this Contest at any time if a computer virus, bug, or other technical problem corrupts the administration, security, or proper play of the Contest.
6) GRAND PRIZE:
There is only one prize. This Grand Prize consists of a trip for two made available through Collette Vacations. The prize includes airfare, hotel accommodations and some meals. The total value of the trip/prize is not to exceed $1500. The winner of this prize will be subject to all applicable local, state and federal taxes.
The trip must be taken during 2009. Any part of this prize not redeemed in 2009 will be forfeited.
7) SELECTION OF THE WINNER:
The winner will be determined by a random drawing from all eligible entries, to be held on or before 15 days after the contest end date at the Senior Circle Marketing office. All decisions are final and binding. Odds of winning will be determined by the number of eligible entries received. The winner will be notified by e-mail or regular mail on or about 20 working days after the contest end date. Sponsors are not responsible for and shall not be liable for late, lost, misdirected, or unsuccessful efforts to notify winner.
Winner may be required to sign and return a release of liability, affidavit of eligibility, and, where lawful, a publicity consent agreement. By accepting and/or using prize, winner and guest agree to the use of their names, voices, or likenesses for the purpose of advertising, trade, or promotion without further compensation, unless prohibited by law. If a selected winner cannot be contacted, is ineligible, fails to claim a prize, or fails to timely return the completed and executed affidavit and releases as required, prize will be forfeited and an alternate winner may be selected.
8) LEGAL CONSENT:
By participating in, and submitting an entry to, this Contest, participants agree to release and hold harmless the following from any losses, damages, rights, claims and actions of any kind in connection with the Contest or resulting from acceptance, possession, or use of any prize, including without limitation, personal injury, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy: Community Health Systems or Senior Circle or their affiliates, contest sponsors, employees, officers, directors, shareholders, agents, representatives, parent companies, affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, and legal advisors,
